The Care Manager position provides assistance in identifying member/family needs and gaps in service; assists client by helping to identify and resolve barriers to treatment, such as transportation, medications (getting prescriptions filled), obtaining medical documentation, Medicaid application assistance, connecting with other local agencies and services for meeting basic life needs.
The Care Manager co-facilitates education efforts with designated families and clients in the FIT program or other group options. In addition, this person coordinates case management and community outreach to assist with recovery; facilitates strength-based recovery groups for adult clients; maintains thorough documentation utilizing Electronic Health Record or other documentation methods.
Education or Formal Training:
- Bachelor’s degree in Human Services or related field
- 0-2 years of experience in community services.
This is a full-time position.
