Solvista's Care Coordination is provided in partnership with Integrated Community Health Partners (ICHP). ICHP's mission is to integrate health care resources and services in partnership with patients, providers and communities. ICHP provides comprehensive Care Coordination which addresses medical, social, developmental, behavioral, educational and financial needs to optimize health and wellness outcomes.
Care Coordination provides a person and family a partnership approach to healthcare. Care Coordinators assess individual and/or family needs, goals and strengths. Then they assist with developing a plan to meet desired outcomes. Coordination with primary care providers and social service agencies is also an integral part of ensuring the most successful outcomes and achievement of goals.
The goals of Care Coordination are to:
- Reduce the amount of unmet needs among those we serve
- To increase access to community resources and services
- To help increase overall health and wellness; and decrease over-utilization of unnecessary medical services
- To empower individuals in navigating health care needs
