What should you expect?
We want get to know a little about what you’re looking for in treatment. This information will help us get a better idea of your needs and expectations for the services. This is important to help us find the best match with a treatment provider to meet your needs.
We need basic information to set you up in our system, such as name, phone number, address, and insurance information. All is confidential and will not be shared for any purposes beyond supporting your treatment needs.
Why do we need insurance information? As a healthcare provider we bill insurance for services. If you don’t have insurance, we can help you get care without it. We never want lack of funding to stand in the way of getting treatment.
We believe in a person-centered approach to care. You have the right, as a patient, to be informed about your healthcare. Completing this agreement will help provide needed information and give us your permission to support your treatment needs.
After you complete these three steps, you can expect a call back from one of our enrollment specialists by the next business day to get you scheduled for your first appointment. We’ll help you find a time to meet with a therapist matched to your needs.
